Earle Family Medical Back to clinic

Website Privacy Policy

EARLE FAMILY MEDICAL CLINIC | Effective Date: October 22, 2026

This policy concerns the informational website of Earle Family Medical Clinic, operated by Sudesh Banaji, MD PLLC. It is distinct from our HIPAA Notice of Privacy Practices, which governs protected health information (PHI) created or maintained for patient care and related healthcare purposes.

Public website and technical information

The Clinic’s public website is hosted using Lovable and provides information about services, location, hours, providers, and patient resources. It does not intentionally provide a Lovable-hosted patient intake form. The website host and technical services may process information necessary to deliver the website, such as IP address, browser and device details, page requests, timing, and diagnostic/error information. Technical and tracking information can be sensitive depending on the circumstances.

Insurance verification and patient submissions

When you select Verify Insurance, you will be directed to the Clinic’s approved Google Form operated through its Google Workspace account. Depending on the form, we may request your name, contact details, date of birth, insurance carrier, membership information, and other information needed to verify coverage or coordinate requested care. Some Google Forms document-upload features require a Google sign-in. Responses may be stored in Google Forms, Drive, and restricted Sheets if the Clinic configures them. We use patient submissions only for purposes permitted by applicable law and the HIPAA Notice of Privacy Practices. Submitting information does not guarantee coverage or admission.

Cookies, maps and related tools

The public site may use technical functionality necessary to display and maintain pages. Third-party services used to provide maps or fonts may receive technical information when loaded. The Clinic does not authorize advertising pixels, behavioral retargeting, session replay, or analytics that disclose patient PHI. Whether particular hosting-layer cookies or diagnostic requests are present depends on the deployed configuration and is reviewed under the Clinic’s website security policy. Browser cookie controls may affect some site functions.

Sharing and security

Technical website data may be processed by the website host and its authorized service providers as reasonably required to operate and secure the public website. Patient information submitted to the approved Google Forms environment is accessed only by authorized Clinic personnel and approved service providers for treatment, payment, healthcare operations, or other lawful purposes. Appropriate administrative, physical, and technical safeguards are maintained for PHI. No internet-based service offers absolute security.

Communication and external links

The website may link to Google Forms, Google Maps and other external services, which may be governed by their own notices. Ordinary email and contact links are not emergency channels; please do not send health records or insurance documents through ordinary email. For emergencies call 911.

Rights, updates and contact

Your rights relating to PHI are described in the Clinic’s HIPAA Notice of Privacy Practices. We may update this policy when website or legal practices change and will post an updated effective date.
